Class Withdrawal Form

By filling out and submitting this form, you are requesting to withdraw your child from his/her selected class. We require notice by the 20th of your last month. There will be no tuition refunds for short notice of withdrawal. If you withdraw mid-month, you will not be charged for the next month; however, you will not be refunded for any unused weeks of class remaining in the current month. All make-up tokens will be expired as of your last day of class and cannot be used once you withdraw from classes. Competitive and pre-competitive program athletes, and Dance Company members, must contact your instructors to withdrawal from your current competitive program position.
